[ch 8: page 245]According to the Acas Code, the request should include the following information:
• date of the application, the change to working conditions the employee would like and when they would like it to come into effect;
• what impact, if any, they think the change will have on the business and how they think that effect might be dealt with;
• confirmation that it is a statutory request; and
• confirmation whether a previous request has been made in the last 12 months.
An employer can ask for less information and can continue to operate any existing procedure that does not demand this information as long as the employer’s rules are clear.
Any disabled employee who is making a request under the statutory duty to make reasonable adjustments (see page 166) should make this clear, says Acas.
