[ch 3: page 31]The guide Harassment at work, by public services union UNISON, provides a model workplace survey. In order to maintain confidentiality the survey should not request individual’s names.
It suggests including questions such as:
1. Have you ever been sexually harassed at work?
2. If yes, circle which of the following you have experienced:
• indecent or suggestive remarks;
• questions, jokes, or suggestions about a colleague’s sex life;
• the display of pornography in the workplace;
• the circulation of pornography (by email, for example);
• unwelcome and inappropriate touching, hugging or kissing;
• requests or demands for sexual favours;
• any unwelcome behaviour of a sexual nature that creates an intimidating, hostile or humiliating working environment.
3. Are you currently being bullied or harassed?
4. How often does this happen?
5. Who is doing the harassment?
• supervisor;
• manager;
• colleague;
• the public (clients, patients, customers, contractors, etc);
• union representative;
• other.
11. What action was taken?
12. Have you lodged a complaint or grievance?
13. When was the last act of bullying or harassment? (An employment tribunal claim must be lodged within three months of the last act of harassment).
